Overview Registered Nurse Disease Management - Remote role with Spectrum Healthcare Resources (SHR). This position is remote, working from the nurse’s home, with Hawaii residency and timezone requirements. Responsibilities Perform assessments, education and training to coordinate care for the dependents of Active Duty members and eligible retirees. Qualifications Bachelor of Science in Nursing (BSN) or equivalent nursing degree. Hawaii state nursing license; an additional state license is preferred but not required. Compact licensure is preferred. At least 2 years of experience as a Registered Nurse Disease Manager (RNDM), preferably in remote or hybrid environments; behavioral health experience strongly preferred. Preferred certifications in disease-management areas such as: Chronic Care Professional (CCP) – Health Sciences Institute Certified Health Coach (CHC) – National Society of Health Coaches Certified Diabetes Educator (CDE) – National Certification Board for Diabetes Educators Certified Asthma Educator (AE-C) – National Asthma Educator Certification Board Highly organized, self-directed, able to function in a high-volume environment without distractions. Strong verbal and written communication skills. Proficient with Microsoft Office and strong technical aptitude. Must have access to secure, high-speed internet. Work Schedule Monday – Friday No nights and weekends 8 hours a day Salary Salary Range: $85,000 – $90,000 annually.
