Pay Range: $23 - $25/hour . Employee benefits include health insurance (medical, dental, vision), 401(k), and paid sick leave (depending on work location).

Key Responsibilities:

• Managing a caseload for healthcare members with long-term care needs.
• Monthly and quarterly member contact, including 80% travel. Remote role. Drivers license required.
• Conducting member assessments and documenting notes.
• Completing assessments with members, caregivers, or providers to gather information for care plan development.
• Monitoring service delivery and following up with members, caregivers, or providers through in-person visits and calls.
• Authorizing and coordinating referrals for services.
• Ensuring provider services are delivered without gaps and identifying deficiencies in care plans.
• Assisting in developing informal or voluntary services for members, collaborating with discharge planners, physicians, and other parties to coordinate discharge and care plans.
• Assisting members with filing and resolving complaints and appeals.

Key Requirements and Technology Experience:

• Experience in Long term care and Home visits .
• 2+ years of Care Management experience.
• Experience with electronic medical records.
• Bilingual in Spanish; proficiency in Microsoft Office.
• Educational requirement: Bachelors degree with 2-4 years of related experience.
