Nurses needed to visit patients: Private Duty

Responsibilities:

• Provide nursing care to elderly, Children and/or disabled patients
• Assess patient needs and develop corresponding treatment plans
• Monitor patient status and notify office staff and supervising physician of significant changes
• Communicate with supervising physicians and other members of the care team to provide effective care
• Educate patients, family members, and caregivers on their conditions, diagnoses, medications, and treatments
• Ensure patient records are accurate and complete
• Serve as a patient advocate and provide advice and support to the patient and his/her family
• Record a patient’s medical history accurately
• Monitor patients’ condition including fluid intake and output and compose patient charts
• Provide emotional and psychological support when needed
• Communicate with patients’ family or friends to provide advice, comfort and release instructions

Qualifications:

• Previous nursing experience or Entry-level
• Strong communication skills and ability to teach
• Must be trustworthy, friendly, and dependable
• Strong understanding of nursing, medical skills, and care plans
• Must be self-motivated and able to work well independently
• MUST HAVE A VALID ILLINOIS LICENSE FOR LPN
• Proficiency in Spanish Language an advantage

Skills

• Proven experience as LPN or similar role or Entry-level
• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
• Understanding of confidentiality obligations and nursing best practices
• Excellent knowledge of medical and hospital terminology
• Good physical condition and stamina
• Excellent communication and interpersonal skills
• Compassionate and able to handle stress
• Successful completion of LPN licenses program is a must
• Vent experience
