Job Title: Home Care Registered Nurse
Department: ElderOne
Location: Monroe County
Hours Per Week: 20 hours
Schedule: Eve/Night

SUMMARY:
As a Home Care Registered Nurse, you will play a vital role in enhancing the quality of life for patients through your exceptional skills in teaching, counseling, and providing empathetic care.

RESPONSIBILITIES:

• Patient Care: Conduct initial assessments, develop individualized care plans, deliver prescribed treatments with demonstrated nursing expertise, and coordinate 24-hour patient care.
• Communication: Counsel and guide patients and their families, equipping them with the knowledge to address physical, emotional, and environmental challenges.
• Evaluation & Documentation: Regularly reassess each patient to monitor progress, maintain accurate clinical documentation, and provide support and guidance to home care staff.

REQUIRED QUALIFICATIONS:

• Nursing Diploma required
• Licensed and currently registered as a Registered Professional Nurse in New York State
• Current NYS Driver's license

PREFERRED QUALIFICATIONS:

• 6 months to 1 year experience conducting UAS-NY Community Assessments
• Bachelor's degree in Nursing preferred
• 1 year of experience working with elderly or frail populations preferred
• Access to a motor vehicle

EDUCATION:

AS: Nursing (Required)

LICENSES / CERTIFICATIONS:

• BLS - Basic Life Support - American Heart Association (AHA)
• RN - Registered Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds occasionally, and/or 10 to 25 pounds frequently. Requires frequent walking, standing, or squatting.

$73,528.00 - $98,072.00

The listed base pay range is an estimate for potential base pay for a successful full-time applicant. It may be adjusted in the future and is eligible for additional pay components based on experience, qualifications, specialty, internal equity, location, and contractual agreements.
