Compensation:
• The pay for this position is $35.95 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Highlights:
• This position is a contract assignment with potential to hire permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday–Friday, 8:00 AM – 5:00 PM EST
• Field-based role — must live in Clinton, Clermont, or Hamilton County, OH , and be willing to complete in-person visits to members’ homes or nursing facilities
• Travel required: 50–75% (mileage reimbursed per company policy)
• Fully remote otherwise
Responsibilities:
• Conduct comprehensive assessments of member needs, benefits, and available services through in-person and telephonic interactions
• Develop individualized care plans to address medical, behavioral, and social needs using a holistic approach
• Apply clinical judgment to identify risk factors, barriers, and opportunities for improved outcomes
• Coordinate care management activities, ensuring members receive appropriate services and benefit plan support
• Conduct field visits to members’ homes or nursing facilities to assess health status and promote wellness
• Collaborate with supervisors and multidisciplinary teams to overcome barriers and achieve care management goals
• Maintain compliance with company policies, case management standards, and regulatory guidelines
• Document all case management activities accurately and timely within electronic systems
Requirements:
• Active, unrestricted Licensed Social Worker (LSW) in Ohio (in good standing)
• Minimum 3+ years of behavioral health or social work experience
• Must live in Clinton, Hamilton, or Clermont County and be willing to travel within those regions (50–75%)
• Reliable transportation required (mileage reimbursement provided)
• Strong organizational, time management, and problem-solving skills
• Proficient in Microsoft Office programs (Word, Excel, Outlook)
Preferred Qualifications:
• Prior case management or community-based social work experience
• Experience managing members with chronic or complex health needs
Education:
• Licensed Social Worker (LSW) required
